Turtle diseases

Turtles are among the most popular reptiles kept as pets. Each species has specific environmental requirements and dietary preferences that are crucial to its health. A well-cared-for turtle can live for many years in excellent condition. However, like any animal, turtles can suffer from various diseases or sustain injuries. Most diseases result from improper housing conditions or an inappropriate diet, although some may have causes beyond the owner’s control.

How to prevent turtle diseases

Most turtle diseases stem from inappropriate environmental conditions, poor hygiene, and an improper diet. Disease prevention primarily involves providing species-appropriate housing, proper nutrition, and adequate supplementation. It is also extremely important to keep the enclosure clean and to quarantine new turtles before introducing them to an existing group.

When setting up a turtle enclosure and providing ongoing care, pay special attention to the following key elements:

  • UVB lighting – appropriate UVB output and proper distance from the turtle, regular bulb replacement, correct placement of the lighting within the enclosure, and exposure to natural sunlight (outdoor enclosure).
  • Temperature – selecting the correct heating lamp wattage and maintaining an appropriate basking temperature, creating a temperature gradient within the terrarium, allowing for nighttime temperature drops, monitoring temperatures during key periods of the year (heat waves, the beginning and end of the heating season, and sudden weather changes in spring and summer for turtles kept outdoors), and maintaining the proper water temperature in aquaterrariums for semi-aquatic turtles.
  • Humidity – maintaining appropriate humidity levels and adjusting misting frequency as needed.
  • Substrate – using a substrate that is safe for turtles, minimizes the risk of ingestion, and does not produce dust.
  • Decorative elements – providing flat stones for wearing down claws, larger stones and logs to enrich the environment, and ensuring there are no sharp edges that could injure the animal.
  • Hiding spot – essential even if the turtle rarely uses it. The absence of a hiding place increases stress.
  • Water basin – providing water of an appropriate depth for the species, ensuring safe access, and choosing a basin size that helps maintain proper humidity levels.
  • Food bowl – necessary to prevent the turtle from accidentally ingesting substrate.
  • Feeding – offering a varied diet, adjusting foods seasonally, selecting appropriate plants for land tortoises, limiting fruits and vegetables to occasional treats for land tortoises, and providing the correct proportion of animal-based foods for semi-aquatic tortoises.
  • Supplementation – selecting supplements appropriate for the species and administering them at the correct dosage and frequency.
  • Cleanliness – maintaining a clean enclosure and water source, regularly replacing substrate and water, avoiding the transfer of decorative elements between enclosures, and disinfecting tweezers and hands when caring for different individuals.

There are times when, despite the best intentions and proper care, a turtle may become ill. Just like humans, reptiles can have certain predispositions, periods of lowered immunity, and various ailments that cannot always be prevented. However, proper husbandry and care significantly reduce the risk of many health problems, making it well worth the effort to provide optimal conditions.

Regular observation and veterinary diagnosis

It is extremely important to observe your pet and respond to any abnormalities in its appearance or behavior. Minor symptoms should never be ignored, even if you are unsure whether they are serious. Prompt action is crucial. Many health problems are much easier to treat in their early stages, allowing for a faster recovery before the reptile’s body becomes severely weakened by disease. Conversely, treatment that is started too late can lead to serious complications, additional health issues, and ultimately even death.

In addition to daily observation, it is advisable to perform a thorough physical examination of your turtle from time to time. Check the appearance of its eyes, nostrils, and beak. Examine the inside of the mouth and look for any discharge; the eyes and nostrils should also be checked for signs of discharge. Inspect the skin and shell for injuries, lumps, or other abnormalities. Examine the underside of the shell (plastron) and the claws as well. Monitor the appearance of the feces and ensure they remain normal. Regular check-ups with a veterinarian who specializes in reptiles are also recommended, along with routine fecal examinations for parasites, even if the turtle shows no visible signs of infestation.

Caring for a sick turtle

A sick turtle, once the problem has been diagnosed, often requires care that differs significantly from that of a healthy animal. In such cases, always follow your veterinarian’s recommendations. The first step is to correct any environmental or dietary factors that may have contributed to the condition. Additional supplementation or emergency feeding may sometimes be necessary. Many diseases also require medication, wound care, or even surgical intervention.

Turtles suffering from parasites or skin diseases are often housed in sterile conditions on paper towels. This helps keep wounds clean and dry and allows the substrate to be changed frequently during treatment. A sick turtle requires more intensive care and closer monitoring of both its environment and overall health. In cases involving parasites or infectious diseases, the entire terrarium and any equipment that has come into contact with the affected turtle should be thoroughly cleaned and disinfected. Turtles in very poor condition may require hospitalization.

The most common turtle diseases

Anorexia and loss of appetite

Loss of appetite often progresses to anorexia. It rarely occurs as an isolated problem and is usually a symptom of another disease or underlying issue. While it indicates that something is wrong, determining the actual cause requires careful evaluation of multiple factors. Loss of appetite may result from temperatures that are too low, insufficient basking space, or an inappropriate diet. It can also accompany digestive disorders, gastrointestinal infections, parasitic infestations, excessive stress, weakness, MBD, and many other diseases. In some cases, it may be associated with kidney or liver dysfunction. If the problem persists after environmental conditions have been corrected, you should consult a veterinarian. They can help determine the underlying cause of the appetite loss and recommend appropriate treatment. It is particularly important not to delay veterinary care if the turtle appears visibly weak, has pale oral mucous membranes, or is losing weight rapidly.

Diarrhea

Diarrhea may be caused by internal parasites, an improper diet, or a vitamin A deficiency. During episodes of diarrhea, the turtle’s feces typically have a strong, unpleasant odor, and the animal may become severely dehydrated. More frequent water changes and soaking sessions may be necessary to help maintain hydration. Feeding smaller portions of dry food is also recommended. Diagnosis involves collecting and testing a fecal sample. The turtle’s diet should also be reviewed and adjusted if it is contributing to the problem. Once the underlying cause has been identified, appropriate treatment should be initiated.

Carapace ulcer disease

Carapace ulcer disease can occur in both semi-aquatic and terrestrial turtles. In both groups, infection is caused by various pathogens and is most commonly associated with poor enclosure hygiene, secondary infections, and excessively high humidity. Symptoms begin with minor discoloration and erosions on the surface of the shell, which gradually worsen over time. Putrid cavities develop within the shell and, if left untreated, may extend to the internal organs and lead to sepsis. Treatment involves the surgical removal of affected tissue and appropriate wound care.

Deep carapace necrosis

This is a very serious infection that develops as a result of untreated superficial shell necrosis or fungal infection of the carapace. It affects the bony structures of the shell and may penetrate deeply into the body. This can lead to the formation of a fistula extending into the body cavity, creating a risk of peritonitis. Deep carapace necrosis may appear as a localized abscess or affect a large area of the shell. It is often accompanied by the unpleasant odor of decaying tissue. Depending on the extent of the lesions, treatment can be prolonged and may require hospitalization as well as surgical debridement of the affected tissues.

Shell fungal infections

Shell fungal infections occur most commonly in semi-aquatic species, although terrestrial turtles can also be affected. They are caused by various fungal species that parasitize the skin or shell and frequently colonize previously damaged areas, such as abrasions or wounds. Fungal infections may develop in turtles kept under inappropriate conditions, including excessive humidity, poor ventilation, or heavily contaminated enclosures. Clinical signs vary and may include scaly patches, discoloration, structural changes in the shell or skin, and surface indentations. Each case requires identification of the fungal species involved and the selection of appropriate treatment by a veterinarian. During treatment, it is often necessary to limit the turtle’s contact with water. If left untreated, superficial fungal infections may spread deeper into the body.

Turtle herpesvirus

Turtle herpesvirus is a viral disease transmitted through contact with infected individuals. Characteristic symptoms include conjunctivitis, inflammation of the mucous membranes of the nostrils and oral cavity accompanied by a white coating, necrosis of the mucous membranes of the mouth and esophagus, and nasal discharge. These symptoms often result in loss of appetite, which can progress to severe weakness and anorexia. If any of these signs are observed, prompt veterinary consultation and treatment are essential. There is currently no effective cure for herpesvirus infection, and treatment focuses on prevention and symptom management. Recovered turtles remain carriers of the virus for life and can continue to infect other individuals. Preventive measures include a six-month quarantine period for new turtles, strict hygiene practices, and blood testing for antibodies against the virus.

Vitamin A hypervitaminosis

Symptoms of vitamin A hypervitaminosis include flaking and cracking of the skin on the limbs, neck, or tail. As the condition progresses, blisters and ulcers may develop. The condition is caused by excessive administration of vitamin A. Depending on its severity, skin lesions may require different forms of treatment, so veterinary consultation is recommended.

Upper respiratory tract infections

Upper respiratory tract infections can occur in all turtle species. The most common symptom is nasal discharge, often associated with dusty substrate or a weakened immune system caused by low temperatures, drafts, fluctuations in environmental conditions, or stress. Additional symptoms may include sneezing, loss of appetite, difficulty diving, open-mouth breathing, and swollen eyes. Treatment involves correcting environmental conditions and eliminating sources of drafts and dust. Medication is often required as well.

Skin infections

Skin infections may develop when damaged areas of the skin become infected, as a result of poor enclosure hygiene or excessively high humidity. Affected areas differ from healthy tissue in color and sometimes texture. Treatment usually involves cleansing the skin with an antiseptic solution, although more specialized veterinary treatment may occasionally be necessary.

MBD (Metabolic Bone Disease)

Metabolic bone disease (MBD) is a common condition in tortoises. The symptoms vary widely depending on the severity and progression of the disease. The direct cause of MBD is an abnormal calcium-to-phosphorus ratio in the bloodstream, typically due to insufficient calcium levels. As calcium is drawn from the bones to compensate, severe deficiencies develop throughout the body. These deficiencies disrupt the function of multiple body systems and can result in brittle or softened bones, deformities of the limbs, skull, and shell, neurological disorders, and digestive dysfunction.

If left untreated, MBD often leads to death. The most common underlying causes of MBD include inadequate UVB exposure, outdated UVB lighting, insufficient dietary calcium, lack of proper supplementation, and excessive levels of oxalates, phosphorus, or vitamin C in the diet. Once MBD has been diagnosed, both the environment and diet must be corrected. A veterinarian may recommend additional emergency supplementation to address severe deficiencies. Treatment may take up to six months, and some changes, such as skeletal deformities, are permanent and cannot be reversed.

Swollen or partially closed eyes

Swollen or partially closed eyes may be caused by a variety of factors, including infection, weakness, poor enclosure hygiene, improper lighting, vitamin A deficiency, excessively low humidity, or debris in the eyes. If the problem is caused by dirt or debris, gently rinse the eyes or allow the turtle to soak so it can wash its head. If no foreign material is visible, a veterinary examination is necessary to determine the cause and appropriate treatment.

Internal parasites

Internal parasites are a relatively common problem in turtles. They enter the digestive system through food, including plants, insects, and vertebrate prey. The risk can be reduced by thoroughly washing plants before feeding and purchasing feeder insects from reputable sources. Turtles kept in outdoor enclosures are particularly susceptible to parasitic infections, so regular fecal examinations are recommended for all individuals. Early detection and deworming help maintain the turtle’s health before the infestation becomes severe enough to cause obvious clinical signs. Symptoms may include vomiting, diarrhea, weight loss, loss of appetite, and weakness. Treatment depends on the type of parasite involved and typically consists of administering appropriate oral medications.

External parasites

Mites are small external parasites that attach to the skin and feed on the host’s blood. They can vary in color and are most commonly found in hard-to-reach areas, crevices, sensitive regions of the body, and around wounds, the eyes, and the mouth. Initially, only a few mites may be present, but over time a full infestation can develop. In dry terrariums, mites may sometimes fall into the water bowl and drown, making them easier to detect. Mites are usually introduced from external sources, such as enclosure décor, clothing, or newly acquired turtles.

Treatment is essential, as these parasites can significantly weaken the turtle, cause wounds and secondary infections, and in severe cases lead to anemia or even death. Treatment involves cleaning the skin and providing therapeutic baths. Specialized reptile products designed to eliminate mites are also available. Unfortunately, mites often return because their eggs are highly resistant and may remain hidden in microscopic crevices despite thorough disinfection. For this reason, preventive treatment of the enclosure should continue for several months after the infestation appears to have been eliminated.

Cracked or broken shell

A cracked or broken shell is usually the result of an accident or an attack by a predator, such as a dog. In such cases, immediate veterinary attention and appropriate treatment are required.

Shell pyramiding

Pyramiding occurs most commonly in terrestrial tortoises and is characterized by the shell scutes protruding outward in a pyramid-like shape. These deformities likely have multiple underlying causes. They are most often observed in tortoises fed a diet that provides more calories than their energy requirements demand. Excessive amounts of protein and simple sugars can cause the shell scutes to grow too rapidly, resulting in abnormal elevation. Additional factors that contribute to pyramiding include low humidity, insufficient UVB exposure, and deficiencies of vitamin D3 and calcium carbonate. It should be noted that this shell structure is naturally present in some species, such as the Indian star tortoise (Geochelone elegans) and the Burmese star tortoise (Geochelone platynota).

Algae growth on the shell

Growth of algae is most commonly seen in aquatic turtles kept in excessively polluted water. Algae accumulating on the shell can lead to shedding problems, separation of the keratinized scutes, and secondary bacterial or fungal infections. To prevent these issues, ensure that the enclosure is equipped with an appropriate filtration system and clean the turtle’s shell regularly.

Sepsis

Sepsis, also known as a bacterial bloodstream infection, is a severe and life-threatening condition in turtles. It most commonly develops as a result of localized infections that were not treated promptly or adequately. Symptoms may include redness of the shell or skin, blood streaking within the shell, lethargy, vomiting, loss of consciousness, and many other. Immediate veterinary intervention and treatment are essential.

Superficial carapace necrosis

Superficial carapace necrosis occurs most commonly in semi-aquatic turtles, although terrestrial species may also be affected. The condition is caused by bacterial or fungal infections that develop in areas previously damaged by abrasions or other mechanical injuries. Poor hygiene is the most common contributing factor, although inappropriate temperatures, poor nutrition, stress, and overcrowding can also play a role. Affected scutes become discolored, appearing either darker or lighter than normal, before loosening and separating from the shell. The scutes often detach easily, and initially the turtle may show no other clinical signs. Prompt treatment, often involving antibiotic therapy, is essential to prevent the infection from spreading deeper into the shell and causing more serious complications. During treatment, the turtle’s access to water is usually restricted.

Problems with shedding and retained skin

Problems with shedding most commonly occur in semi-aquatic turtles. Causes may include insufficiently rough surfaces for rubbing, excessively low humidity, infections, vitamin A toxicity, or an improper diet. Retained skin should be carefully removed with assistance. Following this, humidity levels, enclosure furnishings, and the turtle’s diet should be evaluated and adjusted as needed. If an infection is present, veterinary treatment is required.

Overgrowth of the horn plate (Beak)

Beak overgrowth occurs when a turtle is unable to wear down its beak naturally. This is often caused by feeding food that is too soft or excessively chopped. An overgrown beak interferes with normal feeding and can significantly affect the turtle’s ability to eat. Correction should be performed by a veterinarian. The diet should also be modified to encourage natural beak wear and help prevent the problem from recurring.

Overgrown claws

Overgrown claws occur when a turtle is unable to wear them down naturally. Common causes include a lack of hard surfaces in the enclosure, such as flat rocks, insufficient exercise, an enclosure that is too small, or allowing the turtle to roam freely on household floors. Excessively long claws can interfere with normal movement and cause the toes and limbs to bend abnormally while walking, potentially leading to joint problems. The condition of the claws should be monitored regularly and, if necessary, they can be trimmed by carefully filing away the excess length. It is also advisable to improve the enclosure setup so the turtle can wear down its claws naturally through regular activity.

Skin or shell wounds and injuries

Skin and shell injuries may result from fighting between turtles, bites from feeder insects, or contact with sharp objects within the enclosure. Regardless of the cause, wounds require proper disinfection, sterile conditions, and sometimes protective dressings to prevent serious infection. In cases of severe injury or infection, veterinary treatment is necessary. The source of the injury should also be identified and eliminated to prevent the problem from recurring.

Abscesses on the limbs or ears

Abscesses most commonly develop at the site of a previous mechanical injury. They appear as painful, enlarging lumps filled with accumulated pus. In many cases, abscesses require surgical drainage, treatment, and follow-up examinations. If left untreated, they can lead to further tissue infections.

Salmonellosis

Salmonellosis affects both semi-aquatic and terrestrial turtles. A proportion of turtles carry Salmonella bacteria without showing signs of illness. Clinical disease typically develops when the immune system becomes weakened, for example due to prolonged stress or inadequate husbandry conditions. Symptoms can vary and may include diarrhea, pneumonia, and sepsis. Treatment involves antibiotic therapy and measures aimed at strengthening the turtle’s immune system. Complete elimination of Salmonella is rarely possible. In most cases, turtles remain carriers even after clinical symptoms have resolved. Because Salmonella can also infect humans, proper hygiene practices should always be followed when handling turtles, including washing or disinfecting hands after contact.

Uric acid diathesis (Gout)

Gout develops as a result of kidney dysfunction, most often caused by dehydration or a diet that is excessively high in protein. When the kidneys are unable to adequately filter uric acid salts from the bloodstream, these compounds accumulate in the body and eventually crystallize in various tissues and organs. Symptoms commonly include swelling and pain in the limbs. Treatment requires prompt rehydration, immediate veterinary consultation, and appropriate medical intervention.

Empty gut syndrome

Empty Gut Syndrome typically presents as diarrhea accompanied by the passage of poorly digested food. Possible causes include internal parasites, particularly certain protozoa, and previous antibiotic treatment. Veterinary consultation is necessary to identify the underlying cause and either eliminate the parasites or restore the normal bacterial flora of the digestive tract.

Petechiae on the Shell

Petechiae on the shell usually develop when the turtle is suffering from a serious systemic infection and is in poor overall condition. They appear as small red spots on the shell. Immediate veterinary attention and treatment are required.

Vomiting

Vomiting is most often a symptom of a serious underlying condition, such as an internal parasitic infection, food poisoning, sepsis, or salmonellosis. It may also occur when temperatures are too low, resulting in digestive disturbances. If low temperature is not the cause, veterinary diagnosis is necessary to identify the underlying problem and determine appropriate treatment.

Intestinal prolapse

Intestinal prolapse may occur as a result of a blockage within the digestive tract, although it can also be associated with improper diet, incorrect supplementation, or inadequate husbandry conditions.

In some cases, the exact cause cannot be determined. First aid involves isolating the turtle from other animals, preventing the exposed tissue from drying out, and maintaining sterile conditions if bleeding or tissue damage is present. Immediate veterinary treatment is essential.

Stomatitis

Stomatitis may be caused by either bacterial or viral infections. It is frequently observed in turtles that have been in contact with other species. Symptoms include loss of appetite, excessive salivation, redness of the oral cavity, and the presence of a spongy or cheese-like deposit within the mouth. Affected turtles should be isolated from others and examined by a veterinarian, who can prescribe appropriate treatment.

Pneumonia

Pneumonia is a severe disease that progresses rapidly and can result in death within a short period of time. Potential causes include inappropriate temperature or humidity, poor hygiene, and untreated upper respiratory tract infections. Common symptoms include difficulty breathing, open-mouth breathing, abnormal head posture, weakness, dehydration, and lethargy. Immediate veterinary attention and treatment are essential.

Constipation and intestinal blockage

Constipation and intestinal blockage typically manifest as difficulty passing feces and may eventually lead to intestinal prolapse. Possible causes include an inappropriate diet, dehydration, internal parasites, or ingestion of indigestible objects such as stones. As a first-aid measure, a 30-minute soak in lukewarm water is recommended. If this does not resolve the problem and the turtle has not defecated for an extended period, veterinary examination is necessary, as the condition may be more serious.

Poisoning

Poisoning may result from ingesting spoiled food, toxic plants, or plants treated with chemical pesticides. It can also occur when temperatures are too low and food remains in the digestive tract for an extended period without being properly digested. Symptoms vary widely and may include vomiting, diarrhea, seizures, and loss of balance or coordination. If low temperature is the cause, environmental conditions should be corrected. If toxic substances have been ingested, the opposite approach is required: the temperature should be lowered to slow digestion and metabolism, and the turtle should be taken to a veterinarian immediately. In such cases, gastric lavage may be necessary.

Ammonia poisoning

Ammonia poisoning primarily affects aquatic and semi-aquatic turtles. It results from excessive ammonia levels in the water, usually caused by inadequate filtration, infrequent water changes, or poor overall tank hygiene. Symptoms may include lethargy, eye and respiratory problems, loss of appetite, gelatinous feces, and skin disorders. Treatment involves changing the water, correcting husbandry conditions, and closely monitoring water quality parameters. If eye, skin, or respiratory symptoms develop, veterinary treatment is required.

Hip joint degeneration

Hip joint degeneration is characterized by an abnormal gait. Affected turtles may drag the rear portion of the shell along the ground or position their limbs outward rather than beneath the body. Potential causes include slippery substrates, frequent roaming on household floors, overgrown claws, nutritional deficiencies, poor diet, and inadequate UVB exposure. Treatment can be lengthy and may not always result in complete recovery, as some changes are irreversible. Correcting the turtle’s husbandry conditions and diet is an essential part of treatment.

Jaundice

Jaundice is characterized by a yellowish discoloration of the oral cavity, loss of appetite, and excessive hiding behavior. It may be caused by a variety of liver disorders, which can themselves result from improper nutrition and dehydration. Veterinary treatment is necessary, along with identifying and addressing the underlying cause. This may include improving the diet, ensuring appropriate humidity levels, and providing adequate access to water.
